Hartford HealthCare Nurse Practitioner and Physician Assistants:

Join a collaborative and well-established Family Medicine team at Hartford HealthCare as a Nurse Practitioner or Physician Assistant in Primary Care. Practice alongside four physicians, one advanced practice provider, an embedded behavioral health provider, and a clinical pharmacist in a team-based environment dedicated to delivering comprehensive, high-quality patient care. The team is known for its exceptional culture, strong provider collaboration, and efficient workflows that support high-quality patient care. Providers work closely with physicians, nursing staff, medical assistants, and front-office personnel in a collegial environment that promotes both professional satisfaction and clinical excellence.

This is an outstanding opportunity to practice in a team-based setting with access to the resources, technology, and support of one of Connecticut's leading integrated healthcare systems.

Role : Family Nurse Practitioners (FNP) and Physician Assistants (PA)

Location :  Windsor, CT

Setting : 100% Outpatient Primary Care – Family Medicine (Birth+)

Schedule :  Full-time, weekdays only!

Call schedule : No call required

Nurse Practitioners and Physician Assistants Role Scope:

Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.

In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.

Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider’s practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Compensation and Pay Transparency Disclosure:

This is a salaried position. While compensation is presented as an hourly rate for consistency across our postings, this position is paid on a salary basis. The hiring range reflects a good faith estimate of pay for this position as of the posting date. Actual pay will be determined based on qualifications, location, internal equity, and other factors that may include hire bonuses, incentive pay plans, loan repayment, administrative stipends, or relocation.

Qualifications

Requirements:

Licensure : Unrestricted State of Connecticut Medical License (or eligible)

Training : APRN (Family Nurse Practitioner) or PA

Experience :  1+ years’ experience in a primary care or relevant outpatient setting
