Primary Care Nurse Practitioner / Physician Assistant (Geriatrics) – Central California | Fresno Area
Opportunity Highlights:
Full-time Primary Care NP/PA position in an established PACE program
Care for an older adult population using a collaborative interdisciplinary team model
Comprehensive outpatient assessments and ongoing chronic disease management
Coordinate care with physicians, specialists, therapists, and nursing teams
Opportunity to build long-term relationships with a stable patient population
Rotating evening and weekend call

$150K–$190K salary plus annual bonus

Qualifications:
Licensed California Nurse Practitioner or Physician Assistant
Minimum 2 years of NP/PA experience
Experience caring for older adults preferred
Current prescriptive authority and DEA (or eligibility)
