Job description
Oak Street Health seeks a compassionate Nurse Practitioner to provide comprehensive primary care for Medicare patients in underserved communities. In this patient-centered, team-based role, you’ll conduct assessments, diagnose and manage chronic and acute conditions, prescribe medications, and develop preventative care plans.
