Job description

Heartline Staffing is recruiting a Primary Care Physician to provide preventive primary care for adults 65+ within a value-based care model in Jacksonville, FL. Physicians see approximately 15 patients per day and focus on chronic disease management and preventive care, primarily Medicare Advantage.

The role offers a competitive base salary of $250,000–$260,000 with a sign-on bonus and an annual performance bonus of 20% of base salary.
