Job description
Become a part of our caring community Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Jacksonville, FL.
As a Primary Care Physician, you will report to the Associate Medical Director. You will be onsite at the Callahan, FL. clinic.
A day in the life of a Conviva physician
Monday-Friday 8:00am-5:00pm See 12-15 patients per day Longer authorized appointments (45 minutes) You will be on-site.
Required Qualifications
Doctor of Medicine or Osteopathic Medicine Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
Valid, unrestricted license in the state of desired employment (or willing to obtain)
Current, unrestricted DEA license
Screened for TB
Minimum of two+ years of experience in value base care
1+ years of experience managing Medicare Advantage panel of patients
Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

$255,000 - $298,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
