Primary Care Physician - Clinical Team Lead
Location: San Bernardino, California
Compensation: $315,000-$380,000 base + bonus + equity
Schedule: Full-time | Monday-Friday | No weekend clinic
Start: 60-day guarantee

This organization offers a highly coordinated, patient-centered model that integrates medical, social, and behavioral services for individuals who require more support than the typical primary care environment provides. As the Clinical Team Lead, you'll guide a small care team while managing a manageable panel of patients through relationship-driven, longitudinal care.
Key Responsibilities
Lead and support a cross-functional care team delivering comprehensive primary care
Provide both clinic-based and occasional in-home visits to enrolled patients
Emphasize continuity and prevention rather than episodic or volume-driven care
Collaborate closely with behavioral health, social work, therapy, and nursing professionals
Participate in a light call rotation (phone-only, a few nights or weekends per month)
Compensation and Benefits
Base salary: $315,000 to $380,000, depending on experience
Quarterly performance incentives
Sign-on bonus: $10,000
Relocation assistance: Up to $50,000

Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine
Experience or strong interest in working with aging and medically complex populations

Eligible for Medicare and Medicaid provider enrollment in California
