Compensation: $253,643 - $380,464 + bonus opportunity
Monday-Friday schedule | 8:00 AM - 5:00 PM
Outpatient-only primary care
Quarterly Incentive Plan (15% of quarterly earnings)
Long-Term Incentive Eligible - physicians may participate in the Mosaic Health Profit Shares program, designed to reward physicians for contributing to the organization
Day-one PTO (160 hrs), full benefits, 401(k) match, CME (5 days + 2K reimbursement)
What you need: MD/DO, active license, board certified or eligible, DEA license.

The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting.
