Role Responsibilities
Deliver ongoing, thorough care for all ages from children to seniors.
Address chronic conditions, conduct preventive health efforts, manage acute illnesses, and support mental health with a patient-first philosophy.
Perform physical assessments, analyze diagnostic results, and curate individualized care plans.
Work in tandem with care coordinators, mental health professionals, and specialists to deliver integrated care.
Engage in quality initiatives aimed at improving patient outcomes and the efficiency of operations.
Leverage electronic medical records (EMR) ensuring patient documentation is precise and timely.
Provide care through both in-person and virtual channels to meet patient preferences and needs.
Education Requirements
MD or DO from an accredited institution.
Completion of an accredited residency program in Family Medicine.
Certifications
Board Certification in Family Medicine.
Eligible for (or currently holding) an active California medical license.
BLS and ACLS certifications preferred.
Skills and Experience
Proficient clinical and diagnostic skills, emphasizing preventive and proactive healthcare.
