\ NA growing, physician-led post-acute care organization is seeking a dedicated Physician to provide primary care services within long-term care communities across Pennsylvania.
\ NThis role offers a structured weekday schedule, strong interdisciplinary support, and the opportunity to care for a consistent patient population.
What This Role Offers\ N - Monday–Friday daytime schedule
\ N - Minimal call requirements
\ N - No overnight hospital rounding
\ N - A consistent patient panel within LTC communities
\ N - Strong interdisciplinary collaboration
\ N - Administrative and documentation support
\ NWhile some visits are brief and focused, the overall model prioritizes continuity of care and long-term patient relationships — particularly for medically complex seniors.
Clinical Responsibilities\ N - Conduct admissions and new patient evaluations
\ N - Manage chronic and acute medical conditions in the LTC setting
\ N - Perform routine follow-ups and regulatory visits
\ N - Collaborate with NPs, nursing staff, therapists, and facility leadership
\ N - Communicate with familiesand caregivers
\ N - Document encounters usinga company-issued mobile device
\ NThe focus is on quality, coordination, and patient-centered care within a structured post-acute environment.
Schedule\ N - Monday–Friday
- Daytime hours
\ N - Minimal on-call rotation
\ N - No regular nights or weekend rounding
\ NPredictable schedule. Protected personal time.
