n nResponsibilities
nnnYou will report to the Associate Medical Director: Dr. Nabin Agrawal
n
nnYou will work fully onsite at the CenterWell Indy West: 5115 W Washington St Indianapolis, IN 46241
n
nnDeliver comprehensive outpatient care to adult and senior patients.
n
nnMaintain accurate medical records and documentation.
n
nnDiagnose and manage moderately to complex medical conditions.
n
nnCoordinate referrals and collaborate with specialists.
n
nnParticipate in clinical quality improvement programs.
n
nnWork with interdisciplinary teams to ensure holistic care.
n
nnContribute to strategic programs and innovations in care delivery.
n
nnExercise independent clinical judgment in patient management .
n
n nUse your skills to make an impact
nRequired Qualifications
nnnMD or DO from an accredited medical school.
n
nnActive, unrestricted medical license in the state of practice.
n
nnBoard Certification in Family Medicine, Internal Medicine, or Geriatric Fellowship.
n
nnMust be willing to participate in a quarterly limited on-call coverage by phone.
n
nnProficiency with electronic health records and digital documentation.
n
nn2 years' experience managing high-acuity geriatric patients.
n
nnManage Advanced Practice Providers (NPs) in your assigned clinic.
n
n nPreferred Qualifications
nnnSpecialty training in an Outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics.
n
nnExperience working in a Value-Based Care settings.
n
nnProficiency with electronic health records Athena Health.
n
n nWork Environment
nnnCompensation package including sign-on bonus, relocation assistance, and comprehensive benefits.
n
nnOutpatient clinical setting.
n
nnStandard schedule: Monday-Friday, 8:00 AM - 5:00 PM.
n
n nTB Statement
nnThis role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
n$255,000 - $298,500 per year
nThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
