nCenterWell / Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Jupiter, Florida. You will have the opportunity take a hands-on approach, addressing patient needs inside the exam room and look beyond to understand how social determinants of health are affecting health outcomes. We have a unique care model that sets us apart for patients and providers. We are experts in senior-focused primary care with a versatile approach.
nAs a Primary Care Physician, you will report to our Associate Medical Director. This role will be onsite and in-person at the Conviva University Blvd Clinic in Jupiter, Florida.
nA day in the life of a CenterWell physician
nnnMonday-Friday 8:00am-5:00pm
n
nnJoin an experienced team with an established patient panel
n
nnSee 14-17 patients per day . Opportunity to spend more time with patients
n
nnLonger authorized appointments (45 minutes)
n
nnSpend 100% of your time clinically focused on direct patient care, inclusive of patient-facing time and general administrative time as it relates to direct patient care
n
n nRequired Qualifications
nnnDoctor of Medicine or Osteopathic Medicine
n
nnBoard certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
n
nnValid, unrestricted license in the state of desired employment (or willing to obtain)
n
nnCurrent, unrestricted DEA license
n
nnIf selected for this role, you will be required to be screened for TB
n
n nPreferred Qualifications:
nnnMinimum of two+ years of experience
n
nn1+ years of experience managing Medicare Advantage panel of patients
n
nnKnowledge of Medicare guidelines and coverage.
n
nnKnowledge of HEDIS quality indicators.
n
nnFluent in Spanish and English
n
n nUse your skills to make an impact
nAdditional Information:
nnLanguage Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
n$255,000 - $298,500 per year
nThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
