Role Responsibilities

Deliver comprehensive, ongoing healthcare for individuals and families, in all age groups from pediatric to geriatric.

Address chronic conditions, preventive care, acute medical issues, and mental health challenges with a patient-centric methodology.

Perform physical examinations, interpret diagnostic results, and create individualized treatment approaches.

Work collaboratively with care teams, behavioral health specialists, and other healthcare providers to ensure integrated care.

Engage in quality improvement initiatives aimed at elevating patient outcomes and operational standards.

Leverage electronic medical records (EMR) to keep accurate and current patient information.

Provide care through in-person visits as well as virtual platforms to best meet individual patient needs.

Education Requirements

MD or DO from a properly accredited medical institution.

Completion of a residency in Family Medicine from an accredited program.

Certifications

Board Certification in Family Medicine.

Eligibility for (or current possession of) an active New York medical license.

BLS and ACLS certifications are preferred.

Skills and Experience

Exceptional diagnostic and clinical abilities with a strong focus on preventive and proactive healthcare.

Familiarity with value-based care models and enthusiasm for ongoing professional development.

Outstanding communication, collaboration, and interpersonal relationship skills.

Proficiency in using EMRs and telemedicine technologies.
