nWe are currently seeking a Primary Care Physician to join our team full-time at Conviva/CenterWell Primary Care. This role is ideal for a compassionate, experienced clinician who thrives in a collaborative, patient-centered environment and is committed to improving outcomes for adult and geriatric populations.

nThe Physician focuses on outpatient medicine, continuity of care, health maintenance, and disease prevention. Keeps a medical history and medical records. Refers the patient to specialists as needed. Begins to influence department's strategy. Makes decisions on moderately complex to complex issues regarding technical approach for project components, and work is performed without direction. Exercises considerable latitude in determining objectives and approaches to assignments.

n nKey Responsibilities

nnnDeliver comprehensive outpatient care to adult and senior patients.

n

nnMaintain accurate and timely medical records and documentation.

n

nnDiagnose and manage moderately to complex medical conditions.

n

nnCoordinate referrals and collaborate with specialists as needed.

n

nnParticipate in clinical quality improvement initiatives.

n

nnWork closely with interdisciplinary teams to ensure holistic care.

n

nnContribute to strategic initiatives and innovations in care delivery.

n

nnExercise independent clinical judgment in patient management .

n

n nUse your skills to make an impact

nRequired Qualifications

nnnMD or DO from an accredited medical school.

n

nnActive, unrestricted medical license in the state of practice.

n

nnBoard Certification in Family Medicine, Internal Medicine, or Geriatric Medicine.

n

nnMinimum of 2 years' experience in value-based care or managing high-acuity geriatric patients.

n

nnCommitment to improving patient experience and outcomes.

n

nnParticipation in Tuberculosis (TB) screening program.

n

n nPreferred Qualifications

nnnSpecialty training in Family Medicine, Internal Medicine, Med-Peds, or Geriatrics.

n

nnExperience working with senior populations or in value-based care settings.

n

nnProficiency with electronic health records (EHR) and digital documentation.

n

nnStrong communication, collaboration, and interpersonal skills.

n

nnAbility to work independently and adapt to evolving clinical environments.

n

nnExperience supervising Advanced Practice Providers (NPs/PAs).

n

n nAdditional Information

nnStandard schedule: Monday-Friday, 8:00 AM - 5:00 PM.

nPay Range

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

n$255,000 - $298,500 per year

nThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
