Model: Patient Center Medical Home Model
Start date: 60-90 Days
Schedule: Mon - Fri
Patient Load: 18-22 PPD
EMR : Epic
Support: Full Staff
Opportunity to teach Family Medicine residents and medical students
Compensation/Benefits: Salary: 251-277K (Experience based)
