Job description
Role: Primary Care Physician responsible for providing equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.
Responsibilities:
Assess, diagnose, and oversee primary care for patients
Prescribe medications and manage treatment plans
Maintain electronic medical records using Canopy and Greenway
Collaborate with regional and center leadership to achieve health quality goals
Work with practice managers to direct care teams
Perform additional duties as assigned
Qualifications and Skills:
MD or DO with internal medicine or family medicine certification (or eligibility)
Active, unrestricted state license and DEA license
Preferred: Geriatrics fellowship, experience with older adults, value-based care, outpatient primary care, bilingual in Spanish
Motivated to deliver high-quality care, including new graduates
Additional Details:
Full-time role with a typical pay range of $174,070 to $374,920, offering comprehensive benefits focused on colleagues’ well-being. Emphasis on teamwork, safety, and quality in healthcare delivery.
