Job description
A well-established, nonprofit, community-focused healthcare organization in the New Haven, Connecticut area is seeking a full-time Primary Care Physician to join its growing outpatient practice. The Primary Care Physician will provide comprehensive care to patients across the lifespan, including preventive services, chronic disease management, and acute care. This role emphasizes continuity of care, evidence-based practice, and collaboration with a multidisciplinary team in a supportive clinical environment.
Key Responsibilities:
Provide comprehensive primary care services including physical exams, diagnosis, and treatment planning
Manage acute and chronic conditions such as diabetes, hypertension, and respiratory illnesses
Perform routine procedures and preventive screenings
Prescribe medications and therapies in accordance with state regulations
Collaborate with specialists and care teams for coordinated patient care
Educate patients on health maintenance, disease prevention, and lifestyle modifications
Maintain accurate and timely documentation within the EMR
Participate in quality improvement initiatives and population health programs
