Founded as Boulder Hospice in 1976, TRU Community Care is a nonprofit healthcare organization serving Boulder, Denver, and surrounding counties. With a mission to affirm life at every step of the journey with illness and loss, TRU provides a continuum of care through TRU Hospice, TRU PACE (Program of All-Inclusive Care for the Elderly), TRU Palliative Care, TRU Grief Services, and Landmark Memory Care.

Title: PACE Primary Care Physician

Schedule: Monday-Friday, 8am-5pm

Location: Lafayette, CO

Pay: $229,270 to $283,888 per year

On Call: Sun–Wed or Thurs–Sat, depending on rotation. Phone-only; no travel required.

JOB SUMMARY

The Physician works in collaboration with other providers and interdisciplinary teams to deliver primary care and other health services to geriatric participants enrolled in PACE. This position offers the opportunity to build meaningful, long-term relationships with older adults while providing coordinated, person-centered care in both clinic and community settings. The Physician may also participate in the clinical call rotation for TRU Community Care and provide care to hospice patients.

ESSENTIAL DUTIES

Provides diagnosis, treatment, and ongoing management services for PACE participants and hospice patients.

Provides primary care in the clinic and, when needed, in the participant's home.

Responds to urgent clinical needs during regular work hours and participates in the shared, phone-only after-hours call rotation. No travel is required for on-call coverage.

Participates actively in interdisciplinary team conferences, helping to develop and revise participant/patient care plans and serving as a medical resource in determining approaches to individual participant/patient care.

Collaborates with other providers and members of the interdisciplinary team to provide coordinated, person-centered care.

Provides palliative care and symptom management as appropriate.

Participates in the clinical call rotation for TRU Community Care. On-call coverage is provided by phone, with no travel required.

Works in a safety-conscious manner and follows safe work practices to protect participants, patients, coworkers, and self.

EDUCATION AND QUALIFICATIONS

Physician (MD or DO) with 2+ years of primary care experience.

Unencumbered medical license in Colorado.

Current controlled substance registration and DEA registration.

Eligible for or currently possess the required Medicare/Medicaid provider credentials.

Eligible to bill insurance for palliative care and symptom management visits.

Experience working with frail and/or elderly patients preferred.

Must have $1M/$3M professional liability insurance; TRU will reimburse the premium costs.

Must possess a valid driver's license, provide proof of insurance meeting Colorado state minimums, and have reliable transportation for required daytime participant visits.

Must comply with agency health screening requirements, including medical clearance for communicable diseases prior to direct participant contact and proof that all required immunizations are up to date prior to direct participant contact.
