This full-time outpatient opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) in Upland, California, providing physicians with the support, resources, and collaborative care team needed to deliver exceptional patient care.

$253,643 - $380,464 base + bonus opportunity
Monday-Friday schedule | 8:00 AM - 5:00 PM
Outpatient-only primary care
Quarterly Incentive Plan (15% of quarterly earnings)
Long-Term Incentive Eligible — physicians may participate in the Mosaic Health Profit Shares program, designed to reward physicians for contributing to the organization
Day-one PTO (160 hrs), full benefits, 401(k) match, CME (5 days + 2K reimbursement)
What you need:
MD/DO, active license, board certified or eligible, DEA license.
Compensation Range:
$253,643.00 to $380,464.00 The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
