Primary Care Physician Opportunity
Conviva Primary Care, a subsidiary of Humana Inc., is looking for a primary care physician for our clinic in Venice, FL.
As a Primary Care Physician you will work onsite at the Conviva clinic in Venice, FL and report to the associate medical director. Monday-Friday 8:00am-5:00pm

This position includes a sign-on bonus up to $60,000.
Responsibilities
You will deliver comprehensive outpatient care to adult and senior patients.
You will maintain accurate medical records and documentation.
You will diagnose and manage moderately to complex medical conditions.
Required Qualifications
Doctor of Medicine or Osteopathic Medicine
Board certified or board eligible in geriatric medicine, family medicine or internal medicine
Valid, unrestricted license in the state of Florida (or willing to obtain)
Current, unrestricted DEA license, or must obtain
1+ years of experience in primary care, geriatric medicine, or managing chronic conditions
This position is located at 4125 S. Tamiami Trail Venice, FL 34293
Scheduled Weekly Hours: 40
Pay Range: $255,000 - $298,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
