Job description
Job Summary:
Vista Medical Center is seeking a dedicated and compassionate Primary Care Physician to join our team in Waukegan, Illinois. The ideal candidate will provide high-quality, patient-centered care to individuals and families across all ages. This role focuses on prevention, diagnosis, and treatment of acute and chronic illnesses while fostering long-term relationships with patients to promote better health outcomes.
Responsibilities:
Provide comprehensive outpatient care including wellness exams, chronic disease management, and acute care services
Perform physical examinations, diagnose conditions, and create individualized treatment plans
Order and interpret diagnostic tests such as labs and imaging
Educate patients and families on preventive care, health maintenance, and disease management
Collaborate with other specialists and healthcare providers to coordinate patient care
Maintain accurate and timely electronic medical records (EMR) in accordance with hospital policy
Participate in quality improvement initiatives and adhere to evidence-based clinical guidelines
Provide supervision or support for advanced practice providers (NPs, PAs) as needed
Participate in on-call rotation as assigned
Qualifications:
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
Board Certified or Board Eligible in Family Medicine or Internal Medicine
Valid and unrestricted medical license in the state of Illinois (or ability to obtain)
Current DEA license
BLS and ACLS certifications (or willing to obtain)
Excellent communication, clinical, and interpersonal skills
