Job description
Community Health Center Inc seeks a Primary Care Nurse (PCN) to deliver high quality, comprehensive nursing care across the lifespan within a clinical team. The PCN will perform nursing duties, educate patients on preventive and chronic care, and support transitions between care levels using PDSA improvements at CHCI.
