Job description
Community Health Center, Inc is seeking a Primary Care Nurse (PCN) to provide high-quality, comprehensive nursing care to individuals and families across the lifespan. Monday–Friday 8am–5pm, with one evening a week and one Saturday a month, plus a $1,000 sign-on bonus.
In a clinical team, the PCN works at the top of licensure, supports care coordination with PCPs, and educates patients about preventive, chronic, and episodic care. BSN and CT licensure are required.
