Job description
The Primary Care Nurse (PCN) at Community Health Center, Inc. (CHC) in Middletown, CT provides high quality, comprehensive nursing care within a collaborative clinical team.
You will perform a wide range of primary care duties, administer medications and vaccines, educate patients, and support transitions between care levels while adhering to CHCI protocols and OSHA/CLIA standards. This is a full-time, on-site role with a Monday–Friday schedule and occasional evening or Saturday coverage.
