Job description
Hartford Healthcare seeks a Primary Care Nurse to deliver comprehensive, patient-centered care within our Healthcare & Medical Services department. In this role, you will coordinate and provide nursing care across the lifespan, including assessments, immunizations, chronic disease management, triage, and patient education. You will collaborate closely with physicians and interdisciplinary teams to develop care plans, use EHRs for accurate documentation, and support quality and safety initiatives.
