Job description
Hartford Healthcare seeks a compassionate Primary Care Nurse to join our Healthcare & Medical Services team. In this role, you will provide comprehensive nursing care to patients across the lifespan, including assessments, triage, care planning, and chronic disease management. You will administer medications, perform procedures, coordinate referrals, and support providers during exams and treatments. Using our advanced electronic health record, you’ll document care, track outcomes, and educate patients and families on wellness and prevention.
