Job description
Hartford Healthcare seeks a compassionate Primary Care Nurse to join our Healthcare & Medical Services team. In this role, you will provide comprehensive, patient-centered nursing care across the lifespan in an outpatient primary care setting. Responsibilities include patient assessments, triage, coordination of chronic disease management, medication administration, patient education, and care planning in collaboration with physicians and interdisciplinary teams. You will use advanced EHR systems, support preventive care and wellness initiatives, and contribute to quality improvement.
