Job description
Hartford Healthcare is seeking a dedicated Primary Care Nurse to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered nursing care in an outpatient primary care setting, supporting providers in assessments, care planning, and follow-up. Responsibilities include medication administration, immunizations, triage (phone and in-person), patient education, chronic disease management, and coordination of care across our network. You will document in our EHR, collaborate closely with physicians and specialists, and support quality and safety initiatives.
