Tradition Homecare - NY is looking for a compassionate and skilled Private Duty Homecare Nurse to join our dedicated team in Brooklyn, NY. Responsibilities
Provide direct, private duty nursing care to patients in their home environment
Assess, monitor, and document patient health status and progress
Administer medications, treatments, and medical procedures as prescribed
Develop and implement individualized patient care plans
Communicate effectively with patients, families, and healthcare team members
Educate patients and caregivers on health management and best practices
Maintain accurate and timely clinical documentation and records
Requirements
Active RN license in the state of New York
Prior home care, private duty, or similar nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently with minimal supervision
Reliable transportation and ability to travel to patient homes in Brooklyn, NY
CPR/BLS certification required
