Job description
Select Specialty Hospital in Michigan is seeking a PRN Case Manager to support discharge planning, utilization reviews, and care coordination for chronically critically ill patients in a LTACH setting.
The role requires licensure in a clinical discipline (RN/LPN/LV, RT) or SW/MSW per state guidelines and prior discharge planning experience. Availability for at least 2 shifts per month on weekdays (approx. 8 a.m.–5 p.m.) is needed.
