Job description
Mercyhealth is seeking a Physical Therapist for Home Health in Rockford, IL. This PRN onsite role focuses on evaluating patients, creating treatment plans, and guiding home-based recovery.
The ideal candidate is a graduate PT, licensed in WI or IL, with CPR/BLS within 90 days of hire and ability to travel to patient homes. Experience in home health and successful completion of required background checks are preferred.
