Job description
Home Care Network, Inc. seeks a PRN RN Case Manager (RNCM) to join our Northwest Ohio team, coordinating patient care across Defiance, Van Wert, Paulding and nearby counties.
This role emphasizes consistent visits, SOC, admissions, recertifications, and staff supervision within a flexible, travel-friendly schedule. You will report to the NW OH Regional Program Administrator and Director of Clinical & Administrative Operations, with opportunities to grow into part-time or full-time leadership
