Job description
Extendicare Home Health of West Tennessee, part of LHC Group in the Optum family, is hiring a PRN Registered Nurse (RN) to deliver home-based care across West Tennessee. You will collaborate with physicians and caregivers to plan and provide patient-centered services.
This role requires Tennessee RN licensure, a valid driver's license, and the ability to manage care plans, complete OASIS assessments, and support families.
