Source One Management Services, LLC invites licensed NPs and PAs to conduct in-home Health Risk Assessments for Medicare Advantage members in Greencastle, IN. This flexible PRN role lets you set your own schedule while delivering patient-centered care in patients' homes.
You will review medical history, medications and social factors, perform basic health evaluations, and travel locally. Requires active NP/PA license, availability of eight 7.5-hour days per month, and willingness to travel
