Avida Personal Care is a Wisconsin Medicaid-certified personal care agency serving older adults and people with disabilities in Milwaukee County. The organization helps individuals live safely and independently at home through personalized care plans that respect each person’s needs, routines, and dignity. Trained Personal Care Workers support clients with daily living activities such as bathing, dressing, grooming, meal preparation, mobility, household tasks, and supportive home care. Avida Personal Care is bilingual in English and Spanish and is committed to culturally informed care for Milwaukee’s diverse communities. The agency collaborates closely with case managers, discharge planners, managed care organizations, and families to manage assessments, authorizations, and coordinated services.

Role Description

The PRN Nurse is a full-time, on-site role based in Milwaukee, WI. This role provides nursing oversight for personal care services, conducts client assessments, and develops and updates individualized plans of care in alignment with Medicaid and agency standards. Day-to-day responsibilities include monitoring client health status, documenting clinical findings, coordinating with personal care workers, and communicating with families, case managers, and other healthcare professionals. The PRN Nurse will provide education and support to staff, ensure care quality and safety, and participate in compliance and documentation audits as needed. The role may also involve occasional home visits to evaluate care delivery and support care coordination.

Qualifications

Active Registered Nurse (RN) license in the state of Wisconsin and familiarity with Medicaid personal care regulations.

Experience in home care, community health, geriatric care, or disability services, preferably within a diverse urban setting.

Strong clinical assessment, care planning, and documentation skills, including proficiency with electronic health records.

Effective communication and collaboration skills for working with clients, families, personal care workers, and interdisciplinary teams.

Ability to prioritize tasks, manage time independently, and maintain professional boundaries and confidentiality.

Comfort with providing guidance and training to direct care staff and supporting quality improvement initiatives.

Basic computer skills (e.g., email, documentation systems) and reliable transportation for local travel as needed.

Bilingual English/Spanish skills are highly beneficial; cultural competence and sensitivity in serving diverse communities are essential.
