Job description
PRN Physical Therapist (PT) – Home Health
Sweetwater Hospital Association is seeking a PRN Physical Therapist for Home Health to join our healthcare team. In this role, you will work one-on-one with patients in the comfort of their homes, helping them regain mobility, improve function, and achieve their rehabilitation goals.
Education:
Graduate of an accredited Physical Therapy program.
Current, unrestricted Physical Therapist (PT) license in the state of Tennessee.
Current BLS certification.
Valid driver’s license, reliable transportation, and proof of automobile insurance.
Previous home health experience is preferred but not required.
Strong communication, organizational, clinical, and time-management skills.
Ability to work independently and effectively manage a patient caseload.
Supportive clinical leadershipand team environment.

Ongoing education andprofessional development.
