Human Touch Home Health is looking for a compassionate Physical Therapist (PT) for PRN home health opportunities in the rural communities surrounding Fredericksburg, VA.
We're especially looking for therapists interested in patients located within approximately 50 minutes of Fredericksburg , including areas such as King George, Colonial Beach, Bowling Green, Caroline County, Spotsylvania, Locust Grove, Orange, and surrounding rural communities.
If you don't mind a little country road and enjoy working independently with patients in their homes, we have opportunities for you.

What You'll Do
Complete physical therapy evaluations in patients' homes
Develop individualized plans of care
Provide therapeutic exercise, gait, balance, transfer, and mobility training
Help patients improve strength, endurance, and functional independence
Assess home safety and identify fall risks
Educate patients and caregivers
Monitor progress and adjust treatment plans as needed
Complete reassessments and discharge planning
Coordinate care with the interdisciplinary team
Complete accurate and timely documentation using WellSky
What We're Looking For
Active Virginia Physical Therapist License Required
Graduate of an accredited Physical Therapy program
Valid Driver's License and reliable transportation
Comfortable traveling to rural patient locations
Strong clinical and communication skills
Ability to work independently in patients' homes
Home Health Experience Preferred, but Not Required
WellSky Experience is a Plus
