PRN PT IN-HOUSE SNF

nWe are seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehabilitation team at Newport Meadows Nursing and Rehabilitation Center in Christiana, Pennsylvania. As a PRN PT, you will have the opportunity to work with a diverse range of patients, providing high-quality physical therapy services in a supportive and dynamic environment.

nAbout the Role:

nAs a PRN PT, you will be responsible for providing physical therapy services to our patients, including assessment, treatment, and education. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. You will also be responsible for maintaining accurate records, updating patient files, and communicating with physicians and other healthcare professionals.

nResponsibilities:

nnnConduct thorough patient assessments to identify physical therapy needs and develop individualized care plans

nnImplement and conduct physical therapy treatments, including modalities, exercises, and other interventions

n

nnMonitor and document patient progress, adjusting treatment plans as needed

n

nnCollaborate with the multidisciplinary team, including physicians, occupational therapists, speech therapists, and other healthcare professionals

n

nnMaintain accurate and up-to-date patient records, including progress notes and treatment plans

n

nnParticipate in patient care conferences, discharge planning, and other interdisciplinary meetings

n

n nRequirements:

nnnCurrent licensure in the state of Pennsylvania

n

nnMinimum 0 years of experience in acute care, rehabilitation, or related setting

n

nnStrong communication and interpersonal skills
