Job description
Home Care Network (HCN) seeks an experienced RN Case Manager on a PRN basis to coordinate patient care across Southeast Ohio, including Jackson, Vinton, and Gallia Counties. Travel is required within a 30-minute radius daily, with occasional longer trips; mileage reimbursement is provided.
The role supports starts of care, recertifications, ROC, and other assessments, with potential growth into part-time or full-time positions.
