Job description
Safe Life Home Health Care, a Chicago-area home health agency, is seeking a PRN Physical Therapist to provide patient-focused PT services in patients' homes. You will supervise assistants, evaluate functional status, and coordinate with physicians to implement plans of care.
The role requires an IL license, at least one year of home health experience, reliable transportation, and CPR/TB compliance. Travel to patients’ residences is necessary and scheduling is event-driven for flexibility.
