Job Description

The Program Coordinator (PC) is the lead direct support professional foran assigned Supported Living PrograminLakewood, WA and will be responsible providing direct support, hands-on instruction, assistance, and positive direction to individuals with intellectual/developmental disabilities (I/DD) in a manner that ensures their health, safety, successful achievement of service objectives, and personal goals.

This is not a supervisory role. The Program Coordinator reports directly to the Program Director.

Enrich and support the lives of our residents whilemaintaininga safe and healthy environment

Provide companionship and personalized care while encouraging independence and self-care.

Assistclientswith activities of daily living, including dressing, bathing, grooming, toileting, and eating.

Prepare meals and snacks. May include grocery shopping.

Perform light housekeeping and laundry duties.

Assistwith transportation needs for community outings, appointments, and errands

Implement behavior supports and de-escalation during challenging behaviors

Provide mobility and transferassistance, which mayinclude usingmechanical lift devices.

Administrative duties such asassistingwith schedules, one-on-one training of staff,documentationand follow-up of individual and program related issues, and ensuring compliance with policies and Oregon Administrative Rules.

Qualifications

What Makes You A Great Fit:

2+ years of caregiver experience required (experience supporting adults with intellectual and developmental disabilities is preferred)

Valid driver's license required

NAR Preferred

Must be at least 21 years old

HS Diploma or GED required

Have a passion to help others through person-centered care

People-oriented and have a team approach

Dependable, adaptable,andflexible

Goodcommunicationis key in this role.You’llrely on your reading, writing, andcommunicationskills to document care, follow support plans, and collaborate with both internal and external team members.

Able to attend two weeks of PAID in-house orientation and training
