We're looking for a Program Specialist to join our Long Term Care team and help create engaging, purposeful experiences that promote independence, well-being, and quality of life for our residents.In this rewarding role, you'll work closely with residents and the interdisciplinary team to develop individualized programs, coordinate meaningful activities, and support residents in achieving their highest level of function and enjoyment.What You'll DoAs a Program Specialist, you will:Plan, develop, and implement individualized programs tailored to each resident's interests, needs, and goals.Create and maintain engaging activity calendars for your assigned unit.Review fall huddle information to identify root causes and implement appropriate interventions to enhance resident safety.Complete weekly and quarterly fall reporting and maintain accurate documentation.Manage and maintain inventory of durable medical equipment (DME), including wheelchairs, cushions, and slings.Coordinate food and supply requests with the kitchen team at least one week in advance of planned activities and events.Incorporate therapeutic techniques, adaptive devices, and specialized equipment to promote functional abilities, independence, and resident success.Safely transfer and assist residents with ambulation using approved methods and established safety practices.Ensure resident care sheets and care plans are accurate, up-to-date, and revised as needed.Collaborate with nursing staff, rehabilitation services, families, and other departments to provide exceptional resident-centered care.What We're Looking ForThe ideal candidate is:Friendly, positive, and passionate about working with older adults.Organized and able to manage multiple responsibilities effectively.Creative and enthusiastic about designing engaging programs and activities.A strong communicator and team player.Detail-oriented with excellent documentation skills.Committed to resident safety, dignity, and quality of life.Occupational Therapy Assistant Associate Degree REQUIREDOccupational Therapy Assistant certification REQUIREDWhy Join Our Team?Meaningful work that positively impacts residents every day.Supportive and collaborative team environment.Opportunity to use your creativity and make a difference.Competitive pay and benefits.A workplace that values compassion, innovation, and resident-centered care.If you're ready to bring enthusiasm, creativity, and compassion to a role where every day matters, we'd love to hear from you!Apply today and help create moments that enrich lives and make our community feel like home.
