Job description
Job TitleThe position is in the Department of Veterans Affairs (VA), Veterans Health Administration (VHA), Mid-Atlantic Health Care Network (VISN 6), located at the Durham VA Healthcare System (DVAHCS) Ambulatory Care Service, Durham, NC. The incumbent oversees and manages and maintains the Patient Centered Management Module system for all primary care (including but not limited to Home Base Primary Care program, Greenville, and Morehead primary care clinics) as the Principle Facility Coordinator.
