Job TitleProgram Support Asst (OA)DutiesHelpMajor duties and responsibilities include, but are not limited to:Collect and validate information necessary for the appropriate PACT team assignment and enters the necessary data into a variety of databases for use at program, local, and national levels.Remove information from all of the appropriate databases and ensure the patient is properly assigned elsewhere upon patient discharge.Ensures the patient/family member receives written information regarding their status for any needed action.Ensures all processes are accurate, timely, and complete and initiates immediate follow up in response to any incomplete information or inconsistencies among documents.Responsible for the development and maintenance of a database which allows for compiling and tracking of data points necessary for the preparation of reports and the completion of numerous other functions, such as GIS mapping, identifying high priority patients who might need specific assistance in the event of a weather or human- caused emergency, identifying patients on oxygen or ventilation support, and listing of patients who are of Do Not Resuscitate (DNR) status.Serves in a backup role for the numerous programs which are coordinated out of the Non-Institutional Care.Work Schedule: Monday- Friday 8:00am-4:30pmTelework: This position may be authorized for telework.
