We are seeking a dedicated and compassionatePsychiatric Mental Health Nurse Practitioner (PMHNP) to join our rapidly growing team serving long-term care facilities in the Cleveland, Akron, and Canton, Ohio areas. This position offers the opportunity to make a meaningful impact on the mental health and well-being of residents in skilled nursing and assisted living settings.
The ideal candidate will be a skilled and ethical professional who thrives in a collaborative, interdisciplinary environment and is committed to delivering high-quality psychiatric care. This role requires flexibility, strong clinical judgment, and a passion for working with the geriatric population.
Key Responsibilities:
Provide psychiatric evaluations, medication management, and ongoing treatment to residents in long-term care facilities.
Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized treatment plans.
Maintain accurate and timely documentation in compliance with regulatory standards.
Demonstrate a high level of integrity and clinical excellence in all aspects of care delivery.
Minimum Requirements:
Active and unrestricted Psychiatric Mental Health Nurse Practitionerlicense in the state of Ohio.
Board Certification as aPsychiatric Mental Health Nurse Practitioner (PMHNP) .
ActiveDEA license .
Minimum of one year of psychiatric experience required; geriatric experience preferred.
Strong interpersonal and communication skills.
Must be willing and able totravel locally up to 1 to 1.5 hours to assigned facilities.
