Job description
About the Opportunity
Axe Recruiting is partnering with an established, physician-owned psychiatric group practice to hire a Psychiatric Nurse Practitioner on Long Island , working across the practice’s two offices in Islandia (Suffolk County) and Garden City (Nassau County). This is a psychiatry practice that also treats addiction, and much of its work serves people who have spent their careers serving others: 9/11 responders and survivors cared for through a federally funded health program, emergency responders, physicians and other healthcare professionals, and their families, alongside a varied fee-for-service caseload.
The practice is led by board-certified psychiatrists with academic appointments and deep expertise in trauma and addiction medicine, supported by a multidisciplinary team of 13 providers: psychiatrists, nurse practitioners, psychologists, social workers, and mental health counselors. TMS, EMDR, DBT, and buprenorphine treatment are all offered in house.
The Role
This PMHNP role is focused on assessment and prescribing. You’ll conduct psychiatric evaluations and provide ongoing medication management, collaborating closely with the practice’s therapists and psychiatrists on shared patients.
You won’t be starting from zero: a built-up day of existing patients is transitioning to this position, and the current full‑time NP is already adding hours to build a caseload for you before you arrive.
Why Nurse Practitioners Choose This Practice
Time with your patients: follow-ups are booked in 30‑minute slots, not 15
Realistic expectations: leadership explicitly rejects 40‑patient‑hour weeks; targets account for no‑shows, vacation, CME, and holidays
See patients from day one: an out‑of‑network, fee‑for‑service practice means no months‑long wait on insurance credentialing
Real clinical support: weekly case discussions with a supervising psychiatrist at first, moving to monthly as you settle in
Pay the way you prefer: guaranteed W‑2 salary plus incentive, W‑2 hourly, or 1099 per hour of patients seen
Hybrid schedule: four days in the office and one day remote once you’re oriented
What You’ll Do
Comprehensive psychiatric evaluations and diagnostic assessments
Ongoing medication management for depression, anxiety, PTSD, OCD, panic, and related conditions
Treat co‑occurring substance use disorders as part of a general caseload (addiction specialization not required)
Collaborate with therapists and physicians on shared patients and participate in regular case discussions
Maintain timely, thorough documentation
What You Bring
Active New York State Nurse Practitioner license (Psychiatric/Mental Health) with prescriptive authority
PMHNP‑BC board certification
Active DEA registration, or eligibility to obtain one
Several years of post‑certification PMHNP experience (not a new‑graduate role)
Comfort treating all adults 18+, including patients with co‑occurring substance use
Willingness to work across both the Islandia and Garden City offices
Stands Out
Interest in treating adolescents, particularly high school age
Experience with trauma and PTSD, depression, and anxiety disorders
Buprenorphine prescribing experience
Experience with first responder, veteran, or healthcare‑professional populations
Schedule
Full‑time preferred; part‑time of at least 50–60% will be considered for the right candidate. Time is split between Islandia and Garden City (for example, three days in one office and two in the other), with flexibility for Suffolk County residents to spend more time in Islandia. One remote day per week after orientation. Target start before the end of December 2026.
Compensation & Benefits
W‑2 salaried: approximately $169,000 guaranteed base (full‑time), plus a quarterly incentive for clinical hours above target
W‑2 hourly: $120 per hour of patients seen
1099 independent contractor: $130+ per hour of patients seen
W‑2 benefits: employer pays 50% of health insurance premiums; 401(k) with employer match for full‑time employees after one year
Final compensation depends on experience, schedule, and the structure selected.
Our client is an equal opportunity employer.
