Job description
A behavioral health organization in Queens, New York is seeking a Psychiatric-Mental Health Nurse Practitioner to provide community-based psychiatric assessment, medication management, and collaborative care for adults with complex behavioral health needs.
About the Opportunity:
Shift: Day shift
Schedule: Monday to Friday
Hours: 9am to 5pm
Setting: Hybrid (3-4 days field-based, 1-2 days remote)
Responsibilities: Conduct comprehensive psychiatric evaluations and diagnostic assessments
Diagnose mental health disorders and develop individualized treatment plans
Prescribe, monitor, and adjust psychiatric medications in alignment with evidence-based practices and ACT standards
Provide education to clients and families regarding diagnoses, treatment options, medications, and potential side effects
Collaborate with multidisciplinary ACT team members and support crisis intervention, care coordination, documentation, and treatment planning
Qualifications: Minimum of 2 years of experience in psychiatric or behavioral health settings
Master's Degree in Nursing
Active New York State Psychiatric Mental Health Nurse Practitioner (PMHNP) license
Board Certification as a Psychiatric Mental Health Nurse Practitioner
Experience working with individuals with serious and persistent mental illness (SPMI)
Strong knowledge of psychiatric disorders, psychopharmacology, and integrated care practices
Strong clinical judgment and assessment skills
Excellent communication and interpersonal skills
Ability to work independently and collaboratively within a multidisciplinary team
Comfort working in mobile, community-based treatment settings
Desired Skills: Bilingual (English and Spanish)
Previous ACT team experience
Experience with community-based behavioral health programs
Experience treating individuals with co-occurring substance use disorders
Familiarity with electronic medical records, preferably AccuMED
