Job description
21st Century Home Health Services, the Bay Area’s largest home health agency, is hiring a Full Time Physical Therapist Case Manager, Level 2 to visit patients in their homes. You will evaluate and address the needs of individuals to help them live safely and productively in their home environment.
You will supervise 2 Physical Therapy Assistants and collaborate with clinicians to restore function after disease or injury, aiming to minimize disability and help clients reach their maximum
