nPosition Overview
nWe are seeking an experienced Nurse Practitioner with advanced expertise in Functional Medicine to join our team at Relive Health Tempe. The ideal candidate is passionate about hormone optimization, peptide therapy, and medical weight loss — and thrives on building meaningful, long-term client relationships.
nResponsibilities
nPerform comprehensive exams, review labs, and conduct advanced functional health assessments
nDevelop personalized treatment plans (HRT, peptide therapy, IV therapy, supplements, lifestyle coaching)
nTranslate complex medical insights into clear, actionable guidance for clients
nCollaborate with a multidisciplinary team of providers, nurses, and client advocates
nMaintain detailed documentation in the EHR while ensuring compliance with all regulations
nQualifications
nActive NP license in Arizona
n3+ years’ clinical experience with a focus on functional medicine, hormone optimization, peptide therapy, and medical weight loss
nDemonstrated expertise in root-cause medicine and evidence-based wellness approaches
nExcellent communication and client-relationship skills
nStrong commitment to proactive, patient-centered care
