NOW HIRING: LICENSED PT OR PTA
Part-Time | Home Health | Lake Wales | Frostproof, FL & Surrounding Areas
UPLIFT HOMECARE, LLC is seeking a compassionate, dependable, and skilled Licensed Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our growing home health team serving Lake Wales | Frostproof and surrounding communities.
This part-time opportunity allows you to provide one-on-one care in the comfort of patients' homes while working with a supportive clinical team dedicated to improving mobility, independence, and quality of life.
PAY:
$50–$100 PER HOUR
Rate varies based on licensure, experience, and position (PT or PTA).

RESPONSIBILITIES
Provide therapeutic exercises, treatments, and interventions to improve mobility and function.
Monitor patient progress and response to treatment.
Assist with evaluating functional abilities and updating care plans as appropriate.
Complete clinical documentation accurately and within required timeframes.
Communicate effectively with patients, caregivers, and the interdisciplinary care team.
Educate patients and caregivers on exercises, mobility, safety, and treatment goals.
Participate in care coordination and quality improvement activities.
Maintain current knowledge of applicable physical therapy practices and standards.
QUALIFICATIONS
Current Florida PT or PTA license
Graduate of an accredited PT/PTA program
Successful completion of the required national examination
Minimum 1 year of home health experience required
Strong communication, organizational, and documentation skills
Current CPR certification
Valid driver's license, reliable transportation, and current auto insurance
Ability to travel to patient homes throughout Lake Wales, Frostproof and surrounding areas
