Description
Conduct patient assessments in both inpatient and outpatient settings, tailoring individualized treatment plans to their unique needs
Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
Collaborate closely with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimum outcomes
Guide patients and their families through therapeutic goals, discharge planning, and designing home exercise routines
Maintain detailed records of evaluations, treatments, and patient progress as per facility and regulatory standards
Engage in multidisciplinary conferences and care team meetings to ensure seamless care delivery
Education
Doctorate in Physical Therapy (DPT) or equivalent credential from a recognized program
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience in a similar role is strongly preferred
Capability to manage patients with diverse and complex medical challenges
Exceptional communication and teamwork abilities
Familiarity with digital charting and clinical documentation is essential
